CARE” INCLUDING PLANNING FOR ONGOING QUALITY OF LIFE,
TRANSITION ASSISTANCE AND REAL ESTATE SERVICES.

“TOTAL TRANSITION TEAM” |
We are a full service senior assistance and care management
team that takes a holistic approach to helping seniors and their
families through what can be stressful and difficult times.
It is our goal to do what's right for the family and their
loved ones in need. If aging at home makes sense,
we can make all the modifications necessary to provide a safe,
barrier free
home. We can also coordinate all of the services
necessary to keep mom or dad comfortable, safe, and secure.
If the house is too much of a burden to maintain and downsizing makes
sense, we can list the home for sale and find a condo or
retirement community that fits your budget and level of care.

DESIGN A PLAN OF CARE -
Meet with
family members to understand their “Family Dynamics,” making sure
all viewpoints and feelings are considered. This enables us to
create a “Plan of Care” customized to the older adults’ specific
needs and their family involvement.

MEDICAL
ASSESSMENTS -
As part of our
service, we can order a medical assessment through nurse or other
medical professional to assess the psychological and social
condition of your loved one, as well as memory or physical
capabilities.

|
HOME
ASSESSMENTS -
Our “Plan of
Care” includes inspection of the home and making recommendations to
provide a safe environment to maintain quality of life for anyone
with physical disabilities.

|
MEDICARE D
PRESCRIPTION PLAN ASSISTANCE -
Meet with the
recipient, conduct a prescription summary and search through
Michigan’s 41 existing plans for the best plan for his/her medical
needs.

|
LEGAL AND
FINANCIAL PLANNING -
When
necessary, refer you to a qualified lawyer or CPA who is well versed
in elder care law, trust and asset management, as well as lenders
such as reverse mortgage or life insurance.

|
BENEFIT
ASSISTANCE SERVICES - Help
in locating and acquiring Medicare, VA or government benefits that
you may be eligible for.

|
|
HOSPICE CARE
SERVICES -
When needed, put the family in touch with a Hospice service that
will meet their level of care and trust.

HOME MODIFICATIONS - In
order to provide a safe and accessible environment we will
coordinate the installation of ramps, grab bars or other
modifications to help with personal mobility.

|
BARRIER FREE
ACCESSIBLE - Widen
doorways, roll-in showers for wheel chairs, ramps, patient transfer
systems, etc.
|
|
|
ARRANGING
NON-SKILLED HELP -
When extra
help is necessary, we will arrange for help to come into the home to
clean, cook, run errands, or assist with activities of daily living.

|
HOME CARE
SERVICES -
When
professional medical services are needed, we will schedule a nurse,
physical, occupational or speech therapist for skilled medical care.
|
|
|
PRIVATE DUTY
SERVICES -
Arrange for
skilled or non-skilled care, ranging from one hour to twenty four
hours. Available for private pay if necessary.

|
RETIREMENT
COMMUNITY PLACEMENT SERVICES -
When the older
adult or family is ready, we will assist in locating a retirement
community that fits their needs. We have a large comprehensive
database of communities that match every level of need.

DOWNSIZING, HOUSE TO CONDO -
When the house
becomes too much work we can list your home with the largest real
estate company in the world, Coldwell Banker, and find you a
carefree condo that will simplify your life.

|
HOME SALE PREPARATION -
We can get
your house “Ready to Sell”. If it needs patch, paint, or cleaning,
add some curb appeal, or replace the roof if necessary. We can do
whatever it takes to help sell your home for the best price.

|
|
ORGANIZE AND DE-CLUTTERING -
Believe it or not there are
professional organizing companies that will come and help you decide
what you ‘can live without’ and help to organize the rest, which
will help when it comes time to pack and move into your retirement
community or condo.

|
COORDINATE CONTRIBUTIONS -
We will
arrange to have the items that the family can’t use picked up or
delivered to the charity of your choice.
|
|
|
ESTATE SALES OR AUCTION -
We will
arrange to bring in a professional estate sale or auctioneer to
attain the best prices for items being sold.
|
|
|
PROFESSIONAL MOVING SERVICES -
We can
coordinate for you a professional moving company that will pack and
move you at a fair price.
